You will receive a letter through the mail with your first scheduled appointment. If you miss this appointment you will be mailed one additional appointment. A failure to make your second appointment will result in your removal from the wait list and you will have to turn in a new application to be reassigned to the wait list.
What you will need for your Initial Appointment:
Proof of all Household Income and Assets (bank statements, pay stubs, Public Aid letters, Soc Sec/pension letters, etc.)
Birth Certificates and Social Security Cards for ALL household members.
Lease, Rent Receipts and Utility Bills for the last 3 months.
Verification of Housing Defects or Homelessness
Written Statement from your Current Landlord why you have to leave your current home.
Marriage License or Proof of Separation
Proof of Participation or Graduation from educational or training program
and Proof of any other preference points you are claiming.
